It is likely that many dually disordered individuals will be referred to dual disorder treatment from primary addictions or primary mental health treatment. This means that the managed care organization must communicate its assessment criteria for one or the other disorders, and must also agree internally (between addictions and mental health divisions) as to what standardized instruments will be used. Currently, it is rare to find addiction and mental health programs that share a mutual database at any level from private carriers to city, county, State, or even Federal-level systems. Without a common database accepted by both mental health and addictions, managing care by either provider or insurer will be very difficult.

The national institutes on prescription drug addiction dallas abuse, alcohol abuse and alcoholism, and mental health, as well as the programs operated by the Substance Abuse and Mental Health Services Administration, need to provide a model of such a cooperative database. The smart wheelchair component system

While the needs of many individuals with disabilities can be satisfied with power wheelchairs and medicare power wheelchairs, some members of the disabled community find it difficult or impossible to operate a standard power wheelchair. To accommodate this population, several researchers have used technologies originally developed for mobile robots to create “smart wheelchairs” that reduce the physical, perceptual, and cognitive skills necessary to operate a power wheelchair. We are developing a Smart Wheelchair Component System (SWCS) that can be added to a variety of commercial power wheelchairs with minimal modification. This paper describes the design of a prototype of the SWCS, which has been evaluated on wheelchairs from four different manufacturers.

Reliability and validity of the power-mobility community driving assessment

The Power-Mobility Community Driving Assessment (PCDA) is a performance-based measure designed to assess driving performance of individuals using power wheelchairs or handicap scooters in community environments. This article reports the results of pilot testing and an evaluation of the assessment’s reliability and validity. Pilot testing was conducted with a random selection of Canadian occupational therapists working in the area of mobility. Although the response rate was very low, feedback confirmed the utility of the measure and contributed to one substantive scoring revision. Reliability and validity testing was conducted with a sample of 34 drivers. Internal consistency results were positive. Interrater reliability was fair to high but limited by the lack of variability in the scores. Construct validity hypotheses were tested on the relationships between PCDA scores and vision, perception, cognition, and environmental accessibility. Results indicated no relationships between the PCDA and perceptual and cognitive function and only a weak trend for a relationship with environmental accessibility. Concurrent validity was established: PCDA scores were positively associated with the judgments of therapists familiar with the driving performance of participants. In summary, the PCDA has moderate to good reliability, and content and concurrent validity results were found. More research is needed, particularly on the underlying constructs of successful driving performance. At this point, rehabilitation professionals and their clients are urged to use this assessment to establish driving performance rather than relying on assessments of perception, cognition, or environmental accessibility to predetermine whether someone will receive power mobility. Clinicians may find this a useful tool to identify where clients are able to drive safely in community settings, to identify specific learning needs, and, through those, to promote independent living for drivers of power-mobility devices.

Reliability and validity of the six-minute arm test for the evaluation of cardiovascular fitness in people with spinal cord injury.

OBJECTIVES: To design a submaximal arm ergometry test (six-minute arm test 6-MAT), for persons with spinal cord injury (SCI) and to determine the test-retest reliability and concurrent validity of this test. DESIGN: Prospective, exploratory, methodologic study. To determine test-retest reliability, subjects completed the 6-MAT on 2 days, separated by 1 week. Validity was determined by comparing 6-MAT results with peak oxygen consumption (VO2peak). SETTING: Tertiary rehabilitation center. PARTICIPANTS: Thirty subjects with SCI (mean age, 36.3y; 83% male). INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Subjects were evaluated on the 6-MAT and handicap scooters a VO2peak test. RESULTS: All subjects were able to complete the 6-MAT. Test-retest reliability of steady-state oxygen consumption (VO2) and heart rate during the 6-MAT were excellent (intraclass correlation coefficient ICC, .81; 95% confidence interval CI, .58-.92; ICC=.90; 95% CI, .75-.96, respectively). The correlation between VO2peak and 6-MAT VO2 was excellent (r=.92) and the correlations between VO2peak and 6-MAT heart rate (r=.63) and VO2peak and 6-MAT medicare power wheelchairs output (r=.73) were good. CONCLUSIONS: This study showed that the 6-MAT has acceptable values for test-retest reliability and validity. The 6-MAT should be further tested for responsiveness to enhance its use as a clinical tool.

Distribution and cost of wheelchairs and scooters provided by Veterans Health Administration

During fiscal years 2000 and 2001, the Veterans Health Administration provided veterans with more than 131,000 power wheelchairs and medical scooters at a cost of $109 million. This national study is the first to investigate Veterans Health Administration costs in providing wheelchairs and scooters and to compare regional prescription patterns. With a retrospective design, we used descriptive methods to analyze fiscal years 2000 and 2001 National Prosthetics Patient Database data (cleaned data set of 113,724 records). Wheelchairs were categorized by function, weight, and adjustability options for meeting individual needs (e.g., axle position, camber, position of wheels, tilt, and recline options). Results displayed a cost distribution that was negatively skewed by low-cost accessories coded as wheelchairs. Of the standard manual wheelchairs, 3.5% could be considered beyond the customary cost. Regionally, 71% to 86% of all wheelchairs provided were manual wheelchairs, 5% to 11% were power wheelchairs, and 5% to 20% were scooters. The considerable variation found in the types of wheelchairs and scooters provided across Veterans Integrated Service Networks may indicate a need for evidence-based prescription guidelines and clinician training in wheeled-mobility technologies.

Who will actively manage the care of dual disorder clients? If an entity is developed that will manage dual disorder treatment, where will this fit? Will the individual divisions of addiction treatment and mental health treatment oversee such an entity? If so, it is likely that this entity will be so burdened with double and often conflicting bureaucratic requirements that it will not function. If neither mental health nor addictions supervises this new dual disorder entity, who will?

Clearly, the current division of addictions and mental health is inefficient and problematic in terms of information, funding, legal issues, training, and clinical structures. Would it be too radical to propose that the primary entity must be dually competent and that current divisions of mental health and addiction become subdivisions?

Meth = DEATH!

Sooner or later, “methamphetamine means death:” The death that comes from violence or critical illness, the death of future promise for too many young people, and the death of hope for their families. Methamphetamine carries a prognosis that is worse than many cancers. Sometimes, if things go well, I can give them back their lives, but who is going to give them back their futures? I am at the end of the pipeline, and I struggle to prevent those final effects.

We have to remember those human faces behind the statistics. When we work to put together a comprehensive strategy that is measured by the number of arrests, interdictions or labs seized, we should also measure the number of lives saved and the number of people whose futures are restored. It is important to remember this number as we carry on the fight.

DR. RAWSON: Performing medications research, I am aware it is important to develop tools. For those people who are currently addicted to methamphetamine and cocaine, the medications are not yet available, and nothing is on the immediate horizon. We need to look at the resources and knowledge we currently have available and do a better job applying it to the patients who are currently seeking alcohol detox malibu.
